Rural City of Wangaratta – Aged and Community Care are seeking to replace our client management software applications due to being outdated and one that is no longer supported by the vendor. One application supports clients who receive a Home Care Package which is not limited to care planning, case noting, diary of services, case management time and brokering to external service providers.
The other application manages clients under the Commonwealth Home Support Program which also includes client and community care workers management, administration staff, rosters, client schedules. We would like to replace these two applications and supercede with one application that can be used by both teams and include a dedicated client portal for future use.
