This Request for Tender is for establishing a community-based healthy ageing hub within a community or neighbourhood centre in the Central and Eastern Sydney PHN region.

This proposal is to establish one additional healthy aging hub in either Canterbury or Ashfield or Maroubra regions. Applications proposing to cover two regions in place of one will be considered within the specified funding as per sections 8 and 8.1 Part A RFP application guidance.

The hub will promote positive ageing and social connection, support primary health care through provision of social care and assist older people with navigating health, aged care, social and welfare services. CESPHN will provide funding to successful community or neighbourhood centres to implement this innovative model of care aimed at building community capacity around healthy ageing and assisting general practice with provision of social care.
