CESPHN is seeking proposals for a co-design process to re-design of the GP Mental Health Shared Care program. An internal review of the GPMHSC program highlighted several systemic and operational barriers to achieving the aims of the program. To address these barriers, CESPHN is seeking the re-design of the GPMHSC program, and the development of a model based in primary care that facilitates access to preventative and effective healthcare addressing chronic disease for people experiencing severe mental illness.

We are seeking a consultant to undertake this process to develop a service model that can be implemented in the CESPHN region.

The aim is to develop an improved care model that enhances client access to primary care to improve health outcomes.
