This Request for Proposal is for the provision of a Multicultural Health Navigator Service to cover the SA3 areas of Canterbury, Strathfield-Burwood-Ashfield, Rockdale-Kogarah and Hurstville in the Central and Eastern Sydney PHN region.

This service aims to enhance health equity by bridging gaps in health service access for members of culturally and linguistically diverse communities. Bi-lingual navigators will link people with appropriate services and help to improve health literacy and an understanding of the Australian health and social systems.

The multicultural health navigator service will use a person-centred social prescribing approach empowering people to navigate the Australian health and social systems to assist service access to meet client needs. This may include, but is not limited to, services addressing physical and mental health, financial counselling, housing, employment, gambling, and loneliness.

The objectives of the Multicultural Health Navigator Service are to:

- Improve access to health and social services

- Increase literacy and confidence in navigating the health care system

- Bridge the gap between community members and primary health services and other community supports

- Empower community members to connect with others, navigate and access the Australian health and welfare system

Central and Eastern Sydney PHN (CESPHN) invites eligible applicants to apply to deliver a Multicultural Health Navigator Service.
