Murray PHN invites providers to submit a proposal outlining their interest and capacity to adopt enhanced models of care that supports a multidisciplinary team with the requirements of this RFP

Murray PHN is providing $1 million of funding through its General Practice Investment Strategy, to support primary care to deliver services to their community.

Local general practices, including medical services in Aboriginal Community Controlled Health Organisations, are invited to apply for up to $100,000 to adopt enhanced models of care that also strengthen the health system in supporting a multidisciplinary team.

The funding will be awarded to up to 20 general practices to the value of $50,000-$100,000 to resource general practices to implement or embed models of care focusing on cardiovascular disease (CVD), respiratory disease and/or diabetes complex care coordination, including early intervention.

Service Providers must have a Practice that is based in the Murray PHN Catchment area in order to meet the mandatory requirements of this RFP.
